Are you over 65?

Would you like to get out and about more and have the opportunity to attend social activities or make new friends?

Do you have concerns regarding your health and wellbeing?

Are you finding everyday activities increasingly difficult?

If so, you may be interested to know about the Community Innovations Team. The team includes a nurse and an Occupational Therapist who will help you review your situation, suggest ways to make things easier for you, and then help you to follow up these suggestions. Our Community Support Workers will then assist you by linking you up with voluntary groups and activities. They will also support you with a range of other needs you may have as well as keeping in touch to make sure things are working well. At the moment there is a team available for the Havant PO9 area.

We aim to help people over 65 who are at risk of losing their independence, but who currently do not meet the criteria for support from Hampshire County Council’s Adult Services Department.
